Aftercare Coordinator

POSITION SUMMARY:
This FULL-TIME position ranges from $16.00-$18.00 per hour depending on experience and is a remote position. The Aftercare Coordinator is responsible for assisting with non-clinical duties for all phases of follow up after inpatient services, and related duties as assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Follows up on members who are identified by clinical operations as needing aftercare follow up. Verifies compliance with initial follow-up appointments.
- Provides assistance with referral to a provider if initial appointment is not attended.
- Assists members in coordinating outpatient follow-up.
- Checks provider availability.
- Documents results of follow up calls in system.
- Completes non-clinical assessment tools relating to after care or HEDIS activities.
- Calls members and providers and gathers discharge and other non-clinical information from providers/facilities as scripts and workflow allow.
- Develops and completes reports for completion of outstanding after care activities as needed internally and as requested by clients.
- Assists and provides non-clinical activities related to HEDIS expectations as determined by quality management leadership.
- Assists quality management staff with non-clinical work tasks as needed.
Education and Experience Requirements:
- Requires a high school diploma or GED and experience in customer service and/or related field; or any combination of education and experience which would provide an equivalent background.
- Experience with office software such as Word, Excel, and PowerPoint preferred.
- BA/BS in health related field preferred.
Physical Requirements:
- Possible prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift and move up to 25 pounds at times.
- Standard hours of work Monday through Friday, 8:00 am – 5:00 pm.
