POSITION SUMMARY:


 


We are currently recruiting a Program Admin IV. This full time, position pays $37.00 an hour and is in Rockville, MD.


 


 


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Consults with village leaders on specific situations related to members health and well-being

  • Develop necessary forms and policies in collaboration with AAA team

  • Provide home visits with a village member to perform a biopsychosocial assessment

  • Plan and execute a plan of care when appropriate, in collaboration with the village

  • Maintain notes and reports on members served

  • Offer educational presentations to villages on aging and care management related issues

  • Promote services to the villages

  • Evaluate program outcomes

  • Related duties as assigned


 


Required Qualifications :

Required Skills/Abilities:



  • Excellent public speaking and communication skills.


·       2-3 experience with Microsoft Office suite  



  • Valid Driver’s License and access to secure stable, private transportation.

  • Excellent organizational skills and attention to detail.


·       Ability to work independently; must be self-motivated.


·       Excellent verbal, written, and listening communication skills.


·       Ability to understand and adhere to the duties, methods, and procedures required by the position.


Education and Experience:


 


·       Master or Bachelor’s degree in Human Service, or related field, required.


·       2-3 years of experience working with older adults and/ or individuals with disabilities


·       Extensive knowledge of public and private health care and social services resources in Montgomery County and the region


·       1-2 years’ experience working with case management database platforms


·       Aging life care management experience strongly desired


·       Current social work licenses or similar health care credentials, certifications and recognized by the State of Maryland.


·       Valid Driver’s License and access to secure stable, private transportation.


 


Physical Requirements:


 


·       Prolonged periods of sitting at a desk and working on a computer.


·       Standard hours of work Monday through Friday, 8:30 am – 5:00 pm, however evening and weekend hours will be required. 


·       Travel will be required.
