Schedule: Monday–Friday, 37.5 hours per week
Hourly Rate: $23.50
Position Overview:
The Behavioral Health Case Manager II will serve as a key member of the interdisciplinary PROMISE program team, providing care coordination for individuals with severe and persistent mental illness, including co-occurring substance use disorders. The incumbent will be responsible for conducting assessments, developing and monitoring individualized recovery plans, coordinating placement in appropriate levels of care, and ensuring service delivery meets established standards.

Key Responsibilities:
- Serve as a Care Manager for assigned beneficiaries.
- Conduct comprehensive assessments to determine needs and appropriate levels of care.
- Collaborate with team members to develop individualized recovery plans.
- Coordinate and monitor care services with a wide network of community providers.
- Conduct community visits and follow-ups with program participants.
- Maintain accurate documentation and charting in accordance with program standards.
- Serve as a liaison between beneficiaries, providers, and stakeholders.
Minimum Qualifications:
- Bachelor’s Degree in a related field (e.g., behavioral health, social work, psychology).
- Valid driver’s license and ability to operate a motor vehicle.
- Strong communication, documentation, and interpersonal skills.
- Experience in mental health or human services is preferred.
