POSITION SUMMARY:




This FULL-TIME position ranges from $32.00-$35.25 per hour depending on experience and is located in Hanover, Maryland. The Case Manager 1915 I Liaison is responsible for the development, implementation and continued maintenance of population based health promotion and disease management programs, and related duties as assigned.


 


 ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Sets regional strategies related to a standardized approach to preventative health management activities.

  • Serves as community liaison and participates in county and community coalitions that involve strategic planning and implementation of public health programs that benefit low income populations.

  • Analyzes the health education needs of members via the contractual needs assessment and implements as plan to resolve deficiencies.

  • Links members to local health and social services programs.

  • Manages internal and external communications as they relate to health promotion/disease management programs and initiatives.

  • Serves as a liaison with employers, vendors and providers so that programs and data are shared appropriately.

  • Designs and coordinates community activities to increase enrollment and retention of plan members and ensures a positive image of state sponsored programs.

  • Overseeing and managing problem resolution for 1915(i) population

  • Overseeing Provider recruitment and technical assistance

  • Coordinating and participating in meetings with MDH, LBHAs, care coordination organizations and 1915(i) Providers. 



Required Qualifications :

Education and Experience Requirements:


 



  • Requires MA/MS in social work  counseling  or a related behavioral health field or a degree in nursing. 

  • Current unrestricted RN license in applicable state(s), BSN, LMSW, LCSW, LPC, LBA, LMFT.

  • Ability to comprehend medical policy and criteria to clearly articulate health information is strongly preferred. Travels to worksite and other locations as necessary.

  • Licensed mental Provider, preferred experience with Child and Adolescent population.


 


Physical Requirements:



  • Possible prolonged periods of sitting at a desk and working on a computer.

  • Must be able to lift and move up to 25 pounds at times.

  • Standard hours of work Monday through Friday, 8:00 am – 5:00 pm.
