POSITION SUMMARY:


The Community Health Worker (CHW) will focus on enhancing knowledge through education, prevention, and resource navigation for priority populations in a culturally competent manner. This individual will collaborate closely with our health clinics, local partners, and businesses, including schools, faith-based organizations, and other agencies, to improve health outcomes for clients, with a particular emphasis on serving the Latino community. This FULL-TIME position starts at $17-$32 per hour and is located in Chesterfield, VA.




 ESSENTIAL DUTIES AND RESPONSIBILITIES: Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.



  • Works frequently within the district’s health clinics, referring patients for social needs and providing interpreter or translation services.

  • Builds and maintains relationships with residents through door-to-door engagement, involvement in community organizations/coalitions, and performs outreach documentation.

  • Builds relationships with local organizations and leaders to determine health needs of the community and to build trust.

  • Participates in the coordination of regular health promotion activities such as health fairs, organizational events, COVID-19 testing/vaccination events, and other local events.

  • Serves as a community liaison for community members around public health programs and community assessments.

  • Promotes, facilitates, and coordinates health education & promotion trainings, including but not limited to COVID-19 education, REVIVE! opioid overdose response, and/or tobacco education for lay persons.

  • Assists with grant application preparation and reporting, as requested.

  • May participate in community meetings held by local organizations to ensure resident input

  • Participate in developing department goals, objectives, and systems

  • Participate in staff meetings and attend other meetings and seminars as assigned.

  • Recommend new approaches, policies, and procedures, to continually improve efficiency of the department and services performed.

  • Efficiently performs duties as assigned and works as a team within division and company.

  • Perform other related duties as assigned.


Required Skills/Abilities:



  • Excellent verbal, written, and listening communication skills within the English language.

  • Proven, high quality customer service (internal and external customers).

  • Ability to work collaboratively.

  • Solid ability to analyze situations carefully and adopt appropriate courses of action.

  • Ability to understand and adhere to the duties, methods, and procedures required by the position.

  • Excellent organizational skills and attention to detail.

  • Ability to prioritize tasks.

  • Ability to effectively communicate to large and small group audiences.

  • Proficient with Microsoft Office Suite or related software.


Minimum Qualifications


        English/Spanish bilingualism is required.


        Knowledge of: common community health problems; public/private health care practices; community organizing, survey methods and use of data and health education principles


        Skills in: program planning, implementations and evaluation; community assessment and organization; writing reports and program plans; public speaking; managing group dynamics and providing consultation with other professionals and the general public


        Ability to: communicate effectively orally and in writing; utilize excellent interpersonal skills; maintain good working relationships; work independently with minimal supervision; execute problem solving skills; make appropriate decisions and develop program goals and objectives; evaluate program outcomes, impacts and processes.


        Ability to drive State owned vehicle and/or personal vehicle to each locality.


 


Education and Experience:


        Bachelor’s degree in health education, health promotion or related field, or equivalent training and/or lived experience preferred.


        Experience with computers and Microsoft Office software preferred.


        Certified CHW (or can become certified within the first 6 months of employment). 


        Certified Medical interpreter (or can become certified within the first 6 months of employment). 


        Possess a valid, state driver’s license.


 


Physical Requirements:



  • Prolonged periods of sitting at a desk and working on a computer. 

  • Must be able to lift and move up to 25 pounds at times. 

  • Standard hours of work Monday through Friday, 8:00 am – 5:00 pm.

  • Travel may be required up to 10% of the time.
