POSITION SUMMARY:


 


We are currently recruiting for a Human Services Specialist V. This Full-Time position pays    $26.00 per hour and is located Rockville, Maryland.


 


ESSENTIAL DUTIES AND RESPONSIBILITIES:

·         Maintain regular contact with clients


·         Monitor outcomes until clients are in a safe and culturally competent service environment or in a fully recovered condition.


·         Serve as client advocate to ensure clients' needs are met


·         Increase client engagement through follow-up and scheduling assistance


·         Performs miscellaneous job-related duties as assigned.



  • Provides supportive case management services to individuals residing in COVID19 and overflow shelter facilities. 

  • The candidate will assist the household to secure necessary documentation to obtain and maintain employment and permanent housing. 

  • Conducts a comprehensive assessment including Coordinated Entry System screening, the Vulnerability Index-Service Prioritization and Decision Tool and the Acuity Scale to develop an individualized housing plan with each participant detailing the services, resources, and responsibilities required to stabilize the household in permanent housing. 

  • Assist households and collaborative work with housing providers to complete unit applications, including obtaining credit scores and addressing any concerns, completing criminal background checks and addressing any concerns. 

  • Consults with other professionals in and outside county government to coordinate referrals and linkages to necessary resources and services related to housing stability such as employment services, financial literacy and credit repair programs, childcare, parenting, mental health and / or substance abuse services, and medical services when appropriate. 

  • Conducts face to face and tele-conference visits, to monitor household budget, completion of Individual Housing Plan, and provides advocacy to providers as needed. Support administration of Exit Bonus and COVID Rent Relief. 

  • Maintains client records in the Homeless Management Information System (HMIS), County eICM database, and in case record files which documents household eligibility, rental subsidy, and program outcomes


Required Qualifications :

Required Skills/Abilities:


 


·    Experience working with underserved populations


·    Preference will be given to individuals who are multi-lingual


·    Knowledge of local community resources and state and federal programs


·    Abilitto effectivelwork as part of a professional team


·    Mid-level keyboard accuracy; proficiency with Microsoft Word, Teams, Office    365/Teams, and in using the internet.


·    Independenceinusingalaptopcomputer,MicrosoftTeams,andtelephonemessagingsystem.


·    Internet access and skills to use a "smart" phone


·     Ability to understand and adhere to the duties, methods, and procedures required by the position.


·         Excellent organizational skills and attention to detail.


·         Ability to prioritize tasks.


·         Proficient with Microsoft Office Suite or related software.


Education and Experience:


 


·         High school diploma or equivalent.


·         Possession and maintenance of :A valid Class C(or equivalent)driver's license from the applicant's state of residence when required for job-related duties.


 


Physical Requirements:


 


·         Prolonged periods of sitting at a desk and working on a computer.


·         Must be able to lift and move up to 25 pounds at times. 


·         Standard hours of work Monday through Friday, 8:30 am – 5:00 pm, however evening and weekend hours may be required. 


·         Travel may be required up to 10% of the time.
